Subcontractors
Thank you for starting the process to become a Subcontractor for Platinum Renovations!
When you return the documents to us, you will also need to provide these additional documents:
- Copy of your W-9
- Certificate of Insurance (with Platinum Renovations & Consulting, LLC as additionally insured and it must meet all the requirements listed in the downloaded forms)
Please download the PDF forms on this page for the following business types:
Sole Proprietors (with no other employees):
- Download, fill out and sign the Sub-Contractor Packet
- Download, fill out and sign the Declaration of Independent Contractor
All Other Business Types (Partnership, LLC, LLP, Corporation, etc.):
- Download, fill out and sign the Sub-Contractor Packet
When all paperwork is filled out, return it to us with your W9 and Certificate of Insurance to either one of the following:

After you are approved, you will be asked to fill out the Master Contract. You will be given a password to be able to download it from this page or we will email it to you.
The Master Contract will be updated on a yearly basis (usually December for the next year) and all work will be provided to you by Job Work Order based off of the Master Contract.
